Website privacy policy
How this website handles your information. How we protect your medical information as a patient is explained in our Notice of Privacy Practices.
Effective October 14, 2026
What this policy covers
This policy covers Dr. Stride Hip & Joint's website. Once you become a patient, your health information is protected under HIPAA and handled as described in our Notice of Privacy Practices, which controls if the two ever differ.
Information we collect
- Appointment requests. Your name, phone number, email (optional), the reason for your visit, your insurance, the best time to call and how you found us.
- Messages you send us by phone or email.
- Basic technical data our hosting provider records for security and reliability, such as IP address, browser type and pages requested. These logs are kept for a limited time and are not used for advertising.
- One preference stored in your browser if you use the Pause animations button. It stays on your device and is not sent to us.
What we do not do
- We do not use advertising pixels, retargeting tags or session-recording tools on this website.
- We do not sell or share your information for advertising.
- We do not send the details you enter in the appointment form to analytics or advertising companies.
If we add website analytics in the future, it will be configured so that no health information is shared, and this policy will be updated first.
How we use your information
We use what you send us to call you back, schedule your visit, verify insurance, answer your questions and run our practice. Appointment requests are sent through a secure, HIPAA-compliant service that has signed a business associate agreement with us.
Please do not include detailed medical history in the website form. We will collect what we need securely at your visit.
Text messages and calls
If you agree to text messages when you request an appointment, we may text you about scheduling and reminders. Message frequency varies and message and data rates may apply. Reply STOP to opt out or HELP for help. We do not text marketing messages, and consent is not a condition of care.
How we protect information
This website uses encrypted connections (HTTPS). Form submissions are transmitted securely and access is limited to staff who need it to schedule your care. No website is perfectly secure, so please call us at (713) 555-0168 if you prefer not to use the form.
Children
This website is not directed to children under 13. A parent or guardian should request appointments for patients under 18.
Your choices
You can ask us what information you submitted through this website, ask us to correct it or ask us to delete it when we are not required to keep it. Contact our Privacy Officer below.
